Stroke Recovery Service

The Stroke Association have launched a Stroke Recovery Service for Stroke survivors of any age.  This service provides advice and support to patients, their carers and family members

What the service provides:

  • Hospital and Home visits from a dedicated coordinator
  • Assessment to identify your support needs
  • A personalised recovery plan to support your recovery from stroke
  • Practice and emotional support for carers and families
  • Information leaflets and fact sheets in a range of languages
  • Advocacy support where required
  • Access to our network of support groups and longer term support in your local community
  • Introductions and referrals to other local organisations that can help you

How to make a referral:

GP or other healthcare professionals can refer patients or patients can refer themselves by contacting the local Stroke Association Support Coordinator (details below):
