Specialist Weight Management Service and Bariatric Surgery

 

Tier 3 - Specialist Weight Management

Any patients with a history of long term obesity who have tried and failed at commercial or prescribed weight loss programmes can be referred for specialist weight management

Patients considering bariatric surgery must be referred to the 4 Healthy Weight Tier 3 Specialist Weight Management Programme, run by The Bariatric Consultancy Ltd.  The programme includes specialist counselling, dietetics and exercise physiology.  The process can take up to 1 year and requires commitment from the patient

Referral criteria:

1) The patient has either:

  • a BMI of ≥40, OR
  • a BMI of 35–40 and other significant obesity related disease (e.g. type 2 diabetes mellitus or high blood pressure) that could be improved if they lost weight, OR
  • Asian family origin and recent onset type 2 diabetes mellitus (defined by NICE CG189 as diagnosis within previous 10 years) and a BMI of >32.5

2) The patient should also either:

  • have complied with tier 2 weight management services for 6 months but failed to achieve or maintain adequate weight loss, OR
  • has complex disease states or needs that cannot be managed adequately in tier 2 (for example, the additional support needs of people with learning disabilities)

NOTE: The Bariatric Consultancy Ltd. have the following exclusions:

  • Those with suicidal thoughts within the last 12 months
  • Those who have self-harmed within the last 12 months
  • Those with an unstable mental health condition
  • Must be clear of addiction for at least 12 months, e.g. alcohol or recreational drugs

NOTE: Additional eligibility are in place for access for Tier 3 Specialist Weight Management Services across Kent and Medway.  A Kent and Medway wide policy on access to tier 3 Specialist Weight Management Services is currently under development

Please email the above form to bariatric.consultancy@nhs.net.  Alternatively, you can refer using this form via e-RS.

The following details how the GPs can make a referral to the Tier 3 Specialist Weight Management Service:

  • Under Specialty - Select Dietetics
  • ​Under Clinic Type - Select Weight Management

The service Specialist Weight Management Service is the first that comes up.  The service location is The Bariatric Consultancy London

Tier 4 - Bariatric Surgery

NOTE GPs should not directly refer a patient for bariatric surgery.  Patients must first go through the 4 Healthy Weight Tier 3 Specialist Weight Management Programme, run by The Bariatric Consultancy Ltd, who will onward refer a patient to a Tier 4 bariatric surgery provider if this is deemed necessary after they have completed the Tier 3 programme

If GPs are asked by patients (or by a Tier 4 bariatric surgery provider) to undertake any post-op assessments or monitoring such as bloods, please notify the CCG on southkentcoast.ccg@nhs.net since all tier 4 bariatric surgery providers are responsible for carrying out all post-op assessments
