Community Cardiac Rehabilitation Service

 

The Community Cardiac Rehabilitation Service, run by Kent Community Health NHS Foundation Trust (KCHFT), is a professional, multi-disciplinary team of nurses, physiotherapists, exercise physiologists, counsellors, cardiac rehab assistants, administrators and volunteers who work with a wide range of cardiac patients across east Kent. Their aim is to empower people to return to good health, helping them to make lifestyle changes, while maintaining or improving their quality of life, who see patients in their own homes, outpatient clinics in health centres and GP practices and in community venues

What the service offers

The service promotes secondary prevention through lifestyle changes and offer physical, social and psychological rehabilitation through a range of options

The service offers:

  • An eight week exercise programme
  • A wide range of supervised exercise opportunities that cater for individual needs either in the home or community-based
  • Evidence-based medical management
  • A comprehensive education programme addressing risk factors and helping people to achieve their goals
  • Stress management and psychological support
  • A chance to meet other people who are going through similar experiences
  • Onward referral to other health care professionals

The 8 week rehab exercise programmes are run in 5 areas:  Thanet, Canterbury, Ashford, Dover, Folkestone

Cardiac rehab nurses cover the Thanet, Ashford/Shepway, Dover/Folkestone, Deal/Sandwich, Canterbury/Whitstable/Herne Bay areas

Eligibility

Conditions accepted (depending on severity) include:

  • Myocardial infarction
  • Acute coronary syndromes
  • Heart bypass
  • Angioplasty
  • Heart failure
  • Valve surgery
  • Implantable cardioverter defibrillator (ICD)
  • Angina
  • Other cardiac surgery
  • Pacemaker

Advice and Guidance is being made available for all specialties, and is being provided by consultant specialists at East Kent Hospitals.  To make a request or to check to if a query has been answered, you will need to log in via the electronic Referral System (eRS)
